ACE Foundation Programs
While you may shiver at the idea of an insurance plan, ethics committee, licensing board, or even a client requesting your session notes, this is happening more frequently — even to therapists who don’t work with insurance. Poor notes may even lead health plans to refuse claim payments or ask for money back. Well-written records may be your only defense if a client complains. And the quality of your notes can also be a deciding factor in whether a client gets necessary treatment or disability.
But if you are like most therapists, you aren’t proud of your notes, and you never received training in how to write them. And you may not realize how your current note-keeping habits can hurt you – and your clients.
This webinar will teach you how to write BRIEF notes that satisfy legal, ethical, and insurance expectations, so that you’ll feel confident if you receive a records request. You’ll be introduced to 3 note templates you can use immediately. And we’ll analyze 8 sample progress notes, so you will get the rare opportunity to see what “good” progress notes look like.

The Advanced Clinical Education (ACE) Foundation of the NYS Society for Clinical Social Work, Inc. is recognized by the New York State Education Department’s State Board for Social Work as an approved provider of continuing education for licensed marriage and family therapists #MFT-0028; licensed mental health counselors #MHC-0045; licensed psychoanalysts #P-0017; licensed psychologists #PSY-0121; and licensed social workers #SW-0056.
Out-of-state providers are welcome to request a certificate upon completion of the presentation evaluation but must check with their own state licensing boards to inquire about whether or not the certificate is recognized as transferrable for CEs in their state’s jurisdiction.
By the end of this continuing education activity, participants will be able to:
1. Identify 2 ways that keeping vague, late, or non-existent progress notes can harm therapists and clients
2. Identify 5 common health plan documentation requirements
3. Define “medical necessity” for care, a concept used by health plans to approve care
4. Identify legal issues related to altering notes or writing notes long after a session

Barbara Griswold, LMFT is the author of Navigating the Insurance Maze: The Therapist’s Complete Guide to Working with Insurance – And Whether You Should, now in its 9th edition. She authors an online blog called “What EVERY Therapist Should Know About Insurance.” Her experience includes 33 years in private practice in San Jose, CA, and two years serving on a California professional association state Ethics Committee.
