Increasingly, clinical social workers have been grappling with the corporate take-over of health, mental health and substance abuse treatment. Often disguised as non-profits, these entities cultivate strategies aimed at diffusion of professional identity and solidarity demanding allegiance to the fiscal needs of the overall “industry.” Clashes in practice standards as well as ethical challenges involving profit margins that take precedence over clinical judgment and recommendations are pervasive. Consequently, poor, minorities, the working class, and much of the middle class are now underserved with most of the marketing targeted toward the wealthy. Most clinicians in general do not obtain adequate preparation to critique these new business priorities. As a result, clinicians are placed in compromising positions involving possible breaches of ethics. This also applies to private practice, when clinicians are looking for placement. Particular emphasis will be on examples from the substance abuse industry and how a clinician might help prioritize clinical needs over profit.
